Our Sacramento Fire Department team is eager to help you with your request. To request fire and/or medical incident records, please follow these steps:

Request fire incident reports

Fire Incident Reports are available through the City of Sacramento’s City Clerk’s Office. While a written request is not required, public record requests should be in writing so that City staff can more accurately identify the records sought and process the request more efficiently. Please be as specific as possible when making your request; include dates and times, if applicable, and any details regarding the information you are requesting. For your convenience, these requests may also be made by any of the following methods:

Requests for records may be delivered in person by depositing requests in the City Clerk’s drop-box located on the 1st floor of City Hall. (Only guests with appointments are granted access up to the 5th floor.)

Request medical records

Patient Care Reports (PCR) and corresponding medical billing information are available upon request through the Emergency Medical Services (EMS) division’s billing office.

Third party medical records request

In an effort to streamline the processing of medical incident record requests and enhance the Fire Department's ability to provide timely and exceptional customer service to those requesting records, the Department has now contracted with ChartSwap to fulfill these requests. What you will need to register:

  • Completed and patient signed form (online form through ChartSwap) 
  • HIPAA Authorization to Release Medical Records 
  • Valid I.D. (California Driver's License, California I.D., Military I.D. or U.S. Passport) 
  • If the patient's signature is not available, legal documentation such as a subpoena, power of attorney, or letter of representation is required and must accompany the request.

Medical document pick-up is no longer available at the Fire Department. If you have any questions regarding submitting a request through ChartSwap or any other questions, please contact the Custodian of Records at (916) 808-0315.
